The RN Case Manager is responsible for planning, coordinating and facilitating service for a patient. It is important that Registered Nurse Case Managers focus on high quality medical services and efficient processes for the patient and organization.
A Registered Nurse Case Manager's duties consist of:
Communicating with the patient and their family members to determine their needs.
Developing strategies to satisfy the patient’s needs.
Providing support to the patient and their family.
Help in locating the most affordable method of care.
Following up to make sure the plan is meeting and exceeding the needs of the patient.
One year of experience as a Registered Nurse Case Manager within the past two years.
Current RN License in good standing.
Current Basic Life Support through American Heart Association.
